About you

The Discharge Planner is responsible for providing safe, effective and appropriate patient care in a patient-focused environment.  The discharge planner is responsible for co-ordinating complex patient discharge requirements as a part of a multidisciplinary team. 

We are looking for an enthusiastic Registered or Enrolled Nurse with a passion for patient safety and care delivery.  An autonomous work ethic and ability to work effectively in a team with refined time management skills would be required.  Previous experience in similar roles would be beneficial although not essential, however a minimum of three years nursing experience is required.   

Key Responsibilities

  • Coordinating complex patient discharges from hospital
  • Liaising with the multi-diciplinary team and outside agencies
  • Educating staff and patients regarding care needs and services available

What you bring 

  • AHPRA Registration
  • Demonstrated recent clinical experience in an acute care and/or hospital setting
  • Ability to work unsupervised, proficiently and in an organised manner during periods of high demand and activity
  • Demonstrated ability to work effectively as a member of a multidisciplinary team
  • Demonstrated communication, interpersonal skills and customer service skills
  • A desire to join a team committed to the care of others in a mission based organisation
