About the role
The Statewide Progressive Neurological Disease Service (SPNDS) at Calvary Health Care Bethlehem (CHCB) is a publicly funded, not for profit hospital in the southern metropolitan region of Melbourne. SPNDS offers a range of multidisciplinary assessment, education and management services, co-ordinated through a centre-based outpatient clinic for people with Progressive Neurological Diseases (PND). These include but are not limited to: Huntington's Disease, Motor Neurone Disease, Multiple Sclerosis, Muscular Dystrophy, Atypical Parkinson's and Spinocerebellar Ataxias.
The aim of the SPNDS is to enhance the quality of life of patients and families affected by Progressive Neurological Diseases through planning, co-ordinating and delivering the highest levels of multidisciplinary care, to meet their physical and psychosocial health and disability support needs, throughout the different stages of their life and death, with the disease.
We are looking for registered nurses who are interested in developing specialist skills and knowledge in the support and management of people living with PND.
Available: 3-4 days a week Clinical Liaison role. Clinical Liaison work with an allocated caseload and medical team members as a key point of communication for both the patient, family and team. This role will primarily be working with patients who have Motor Neurone Disease.
Key Responsibilities:
Registered Nurses within SPNDS:
- Use 'disease specific' knowledge to provide comprehensive patient assessment including detection and recognition of symptoms, related to treatment, disease process or disease progression and efficiently plan and provide comprehensive person-centre care for people with PND's
- Play a central role in helping to bridge the divide between services both within CHCB and external health, community care, disability and other services, through active care-co-ordination
- Contribute to on-going service design, evaluation and improvement to ensure best outcomes for patients in conjunction with the broader team
- Provide clear communication with patients and their families, considering their individual desire for information and their health literacy levels
- Opportunities to engage in research
What you bring
- A Registered Nurse, with experience in one or more of: Progressive Neurological Diseases, Palliative Care, Mental Health, Disability or Community Health
- Interest in developing disease specific knowledge including a knowledge of community resources and pathways
- Ability to work independently and autonomously in complex situations, within scope of practice, and respond to unknown and unexpected dilemmas
- Well developed communication and computer skills
- Flexibility - adaptable and recognises the value of different opinions and can adapt accordingly
- Motivated - enthusiastic and committed to setting and modelling high standards of work performance
- A desire to join a team committed to the care of others in a mission based organisation
