We are seeking an enthusiastic and motivated Client Coordinator to join the Operations team in the Flinders Far North Service Centre, overseeing and working at our Port Augusta Respite Cottage.
As a Respite Coordinator, you will ensure the services being provided to care for approved Community Home Support Program participants and their carers is provided in a professional and caring manner consistent with Calvary's Mission, Vision and Values and in line with current legislation and Standards. In this position, you will be responsible for assessment and coordination of responsive and flexible quality care that promotes care aligning with participants choice and person cantered approach.
In this role the Client Coordinator will be accountable for:
- Delivering the best support and quality service for Community Home support participants
- Increasing and monitoring of service delivery hours to meet KPI expectations
- Working under Aged Care standards, Department regulations, Calvary policies & procedures
- Ensuring participant reviews and care plans are up-to-date and completed within KPI timeframes
- Escalating Incidents, complaints, feedback, directly to into RISKMAN and escalating to the Service Centre Manager
- Completing CHSP service agreements and budget tools with the participant
- Identifying deterioration and continuous liaison with health professionals and other agencies
- Arranging excursions/ outings in consultation of client's choices and interests
- Day program planning and evaluations of programming to meet individual client care needs
- Overseeing meal planning ensuring dietary requirements are met

- Working hours of Mon- Friday 9:00am – 5:00pm
