About the role
In this full-time role, you will work closely with our Case Managers and Clinicians to provide effective office and telephone support. You will act as the main point of contact between clients, support workers, case managers, and families ensuring that all client care and service needs are promptly addressed. Your responsibilities will include scheduling service requests through our rostering system, communicating updates to the appropriate staff, and verifying that all completed services are accurately recorded
What you bring
- • Certificate III in Community Services or a related field desirable
- • Experience in Aged Care, Disability or Home Care
- • Previous experience in care coordination - preferred but not essential
- • Knowledge and understanding of person centred service delivery
- • Minimum P1 Driver’s License
- • A friendly, engaging and inclusive phone and email manner
- • A desire to join a team committed to the care of others in a mission based organisation
- • Police Clearance within 6 months (or willingness to obtain one)
- • Working with Vulnerable people check essential
