About the role
As a member of the Calvary Home Care team and as an experienced RN, you will provide care to palliative care clients from various short term/long terms programs. You will provide complex care to clients by working independently to support clients and their stakeholders overnight. As a Palliative Care nurse, you will provide high quality patient education, symptom management and End of Life Care in the comfort of a client’s home or residential accommodation.
Key Responsibilities
- Support and education for clients with terminal diagnoses
- Education and support for families and carers
- Administration and preparation of s/c medications and CSCI for symptom management
- General nursing care including simple and complex wound care, IDC/SPC care, PICC dressings
- Accurate/ Timely documentation in all client records.
- Appropriate communication within the clinical team and community palliative care teams and referrers to ensure care is provided.
- Complete initial assessments/develop care plans to ensure best outcome for our clients.
- Monitor and update care plans to ensure appropriate care is provided
- Recognise and respond to clinical deterioration, with appropriate escalation.
- Promoting best practice and initiating quality improvement activities
What you bring
- Current AHPRA Nursing
- Minimum 2 years palliative care nursing experience, community experience desirable
- Post graduate qualifications (or working towards) in Palliative Care
- NDIS Clearance (or willingness to obtain one)
- Police Clearance with in 6 months (or willingness to obtain one)
- Use of own car – KM allowance provided
- Full drivers licence, reliable and comprehensively insured car
- CPR and Manual handling
