About the role

Based at Calvary Kooyong in Caulfield South, Calvary Healthcare Bethlehem is a leading provider of specialist services for people living with progressive neurological diseases and those needing palliative care. Services include an inpatient ward, Community Palliative Care Service, State Wide Progressive Neurological Diseases Service, and NDIS Provider service.

You will work across our service streams, aligning with your experience and skillset. The role will include either casual bank shifts for leave cover which may be single days or consecutive days.  The opportunity is also available for locum fixed term roles if this supports appropriate applicants to accommodate their circumstances. 

You will provide OT recommendations to people with progressive neurological diseases (PND) or other life limiting conditions and their formal and informal support network to support living well. You are part of a supportive and compassionate multidisciplinary team including a wide range of allied health professionals, medical consultants, specialist nurses and external stakeholders.

Your main duties will include:

  • Occupational Therapy assessment and intervention through direct and indirect modes
  • Attending case conferences
  • Supporting NDIS access and navigation of NDIS and aged care systems
  • Working with internal and external stakeholders to provide compassionate care
  • Participation in SPNDS meetings and projects
  • Communication and collaboration with external allied health providers for patient management and education
  • Administrative functions including keeping statistics and managing appointments
  • Documentation of patient notes and reports

About you

  • Awareness of the caseload of people living with life-limiting diseases
  • An OT who enjoys supporting quality of life and problem solving creative solutions
  • You have some experience or interest in developing skills in progressive neurological OT and/or palliative care
  • You have outstanding communication skills
  • Thrive in a dynamic environment
  • Outstanding team player for best patient outcomes
  • You are self-directed, organised, enjoy a balance of team and autonomous tasks
  • You are reflective in your clinical practice
  • You are a person-centred, creative and flexible therapist who is able to work with dignity of risk and respect for individual choice and control
  • You have some knowledge in navigating the interface of palliative care, health and disability to support patients and team members with coordination of care
  • AHPRA registered Occupational Therapist
  • Drivers Licence
