Calvary Health Care Bethlehem is seeking a Grade 1 or Grade 2 Social Worker to join our Highly Experienced Social Work team. This position provides social work services across inpatient, outpatient and community-based programs supporting people living with life-limiting illness and progressive neurological conditions, including Motor Neurone Disease, Huntington’s Disease and Parkinsonian disorders.
This is a permanent part time position based in Caulfield South, Melbourne. You will work within a supportive multidisciplinary environment, providing psychosocial assessment, counselling, advocacy, care coordination and family support across the continuum of care.
- Provide psychosocial assessment, counselling and support to patients and families
- Participate in interdisciplinary care planning and discharge coordination
- Facilitate access to community resources, funding and support services
- Bachelor or Master of Social Work
- Eligible for membership with the Australian Association of Social Workers
- Demonstrated experience in health, rehabilitation, palliative or community settings
- Excellent communication, assessment and care coordination skills
- Current Victorian Driver Licence
