About the role

This position has operational accountability for the inpatient Palliative Care Unit (PCU) and Community Palliative Care Team (CPCT) Social Work team at Calvary Health Care Kogarah (CHCK) and reports operationally and professionally to the Department Head

The Team Leader will provide operational direction, lead, guide and manage the team across CHCK and facilitate the implementation of high levels of clinical expertise in order to provide high quality care to patients/clients and their carers consistent with Calvary and relevant NSW Ministry of Health and South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards. The position contributes directly to the development of policy, with consideration of the strategic context.

About you:
  • Qualifications in accordance with Sub-Schedule A of the NSW (Non-Declared) Affiliated Health Organisations’ Professional and Associated Staff Agreement 2022 for Social Worker and eligible for membership with the Australian Association of Social Workers including demonstrated commitment to ongoing professional development.
  • Extensive clinical experience within palliative care, oncology, aged care or other end of life clinical area in a health care setting Demonstrated ability to provide high-level guidance, leadership, and strategic management of staff, resources, and services, with a strong focus on building team capability and fostering a culture of continuous improvement
  • Demonstrated ability to work as part of a multi-disciplinary team and to apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel, complex or critical nature.
  • Demonstrated ability to initiate, lead, complete and share quality improvement initiatives and service evaluation processes while applying evidence-based practice to enhance clinical outcomes within the designated area. 
  • Excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse range of stakeholders.
  • Ability to work within the Mission and Values of Calvary Health Care Kogarah.
  • A current drivers’ licence (or a willingness to travel in accordance with the demands of the position) and eligible for Working with Children Check (WWCC), National Criminal Record Check (NCRC) and Aged Care Banning Order Check
