About the Role

The role sits within the Innovation and Quality Division, working in partnership across the organisation to lead and support Aged Care Services implementation and embedding of the Living Well Together model of care across BaptistCare sites. Co-designed with our residential Aged Care community, the Living Well Together program is a person-centred, evidence-based approach that supports making a meaningful difference for people to live their best life. The role will ensure that the new model delivers evidence-based practice, consumer choice, ensuring alignment with strengthened aged care standards and improved outcomes.

This role also undertakes referral-based consumer reviews to support evidence -based behaviour management and minimise restrictive practice, whilst building capability across the organisation using their subject matter expertise.

This position supports the investigation, management and reporting of behaviour related externally reportable events.

Travel to residential BaptistCare sites is required to oversee and support The Living Well Together and Montessori staff training and coaching. The initial focus will be Victoria and Tasmania’s residential aged care communities, to support project implementation and staff education.

Key Duties

  • Execution of the Living Well Together project deliverables and bring subject matter expertise in Behaviour Support Planning and Management in line with current best practice and ensure legislative requirements.
  • Act as a conduit between services, operations and Quality and Practice teams to ensure the Living Well Together model is successfully implemented and supported.
  • Liaise with Aged Care transformation team to ensure alignment.
  • Ensure LWT project runs efficiently and to schedule on a day-to-day basis.
  • Assess and communicate risk where decisions have potential impact on the business, consumers or other stakeholders and ensure appropriate actions are taken accordingly.
  • Ensure that the new model encompasses compliance, evidence-based practice ensuring that care standards are being monitored and improved.
  • Guide the process in ensuring staff are recruited, trained, assessed, and equipped to deliver the expected outcomes required by consumers in line with the Living Well Together model and the Strengthened Aged Care Quality Standards.
  • Work to improve operational excellence in critical areas such as quality, cost, consumer service and sustainability in safety and environmental performance.
  • Assist with the development and direction of a program which educates care staff specifically on Dementia, Montessori, Behaviour Support Planning and Restrictive Practice

You will ideally bring the following:

  • Proven leadership capability in a similar role and experience in Aged Care.
  • Qualification(s) in specialised dementia care or equivalent work experience.
  • Detailed understanding of legislation, quality standards and clinical operating procedures in an aged care facility.
  • Excellent communication skills (written, verbal & interpersonal) with ability to promote a consumer- focused service internally & externally.
  • Ability to lead and encourage staff in the provision of quality outcomes in both individual performance & consumer care and promote a culture of self-development and develop suitable options to deliver education with measurable outcomes.
  • Computer literacy (Microsoft Word & basic Excel)
  • Ability to have a positive, flexible approach to the role according to the daily challenges and aligned to the Baptcare values.

Essential:

  • Registered Nurse (AHPRA) qualifications
  • Experience and further education in Dementia Care
  • Must have Driver’s license

Desirable

  • Graduate education or training qualifications or clinical equivalent
  • Minimum 1 year of work experience in Baptcare Residential Aged Care (Preferred)
