About the Role

The Partner in the Community (PITC) Early Childhood Coordinator provides high-quality care and support to children with disability or developmental delay, individuals with disabilities and their families. This position is responsible for providing early connections and early supports for children aged 0-9 years. The Early Childhood Coordinator assists in supporting access to the NDIS when appropriate and developing, implementing, and reviewing NDIS plans. 

Key Duties

  • Facilitate informal and formal assessments for children 0-9 years.
  • Facilitate individual and/or group sessions with participants when required.
  • Provide short term intervention using best practice models and consulting with Allied Health Professionals when required.
  • Assist the individual or nominated contact to develop, implement, review, and monitor their NDIS plans when a funded plan is appropriate.
  • Establish and maintain strong networks with community groups, organisations, government services, and service providers, and link people to these as appropriate.
  • Work alongside individuals and families to build capacity and confidence to live a good life by setting their own goals, exercise choice and control over funded services, seek out and engage in mainstream and universal supports.
  • Supports families to implement plans and monitor their progress.
  • Operates within Baptcare policies and procedures and quality framework.
  • Makes an active commitment to the development and maintenance of cohesive teams both within Baptcare and the NDIA.
  • Participate in meetings, supervision, and staff development.
  • Complete all required training both NDIA LEAP and Baptcare Learning Management System within specified time frames.
  • Attend all mandatory face to face training sessions and workshops.
  • Lead meaningful community engagement and awareness activities and capacity building projects.
  • Communicate in conversations, written work, and in presentations effectively, appropriately, and respectfully.
  • Utilise excellent organisational, time management, and self-direction skills.
  • Work within the NDIS ILC Outcomes Framework and NDIS ECEI principles.
  • Meet KPI’s as specified by the NDIA Statement of Requirements.
  • Complete and maintain all spreadsheets and registers required for KPI tracking.
  • Assist with telephone and face to face enquiries.

 

Key Selection Criteria:

  • Demonstrated comprehensive assessment and planning skills
  • Proven experience working with children aged 0 – 9
  • Outstanding ability to provide early childhood supports and develop strategies in line with best practice
  • Ability to work within complex environments and be outcome focused
  • Demonstrated ability to motivate others and act as a change agent through strengths based, person led practice
  • Proven ability to communicate, build and nurture relationships and partnerships with a range of people and organisations at an individual, service, and community level
  • Demonstrated understanding of, and commitment to, the principles and values of human rights, social justice, and inclusion
  • Demonstrated ability and initiative to effectively use a range of software and learn, embrace, and adopt new systems
