About the Role
The Partner in the Community (PITC) Pathway Coordinator provides holistic guidance, support and advice to people living with a disability and their families, assisting them with navigating the National Disability Insurance Scheme (NDIS) as well as connecting them in with other supports that may be available in their community. The Pathway team operates under a hybrid model, working collaboratively to support individuals across our regions in South Australia and Tasmania.
Key Responsibilities
- Build authentic relationships with community members and local organisations
- Support individuals to identify their goals, interests, and aspirations
- Connect people with existing community resources and networks
- Facilitate introductions and encourage natural community connections
- Maintain accurate records and contribute to team learning
What We're Looking For
- Strong interpersonal and communication skills
- Experience in community development, social work, or related field
- Genuine commitment to person-centred practice
- Ability to work flexibly and autonomously
- Current driver's licence and willingness to travel locally
- Understanding of diverse community needs and cultural sensitivity
Mandatory
Demonstrated understanding of person-led practices including experience and commitment to working with and delivering high quality services to people with disability, children with disability or developmental delay, their families, carers, and local communities
Able to demonstrate an understanding of appropriate behaviours when working with children and vulnerable people
Proficiency in providing informed, meaningful, and relevant messages when communicating with customers, community members and NDIS participants
Ability to provide customers with high quality service and appropriate referrals based on their personal circumstances and preferences
Demonstrated ability to work within complex environments and be positive when faced with challenging situations or conversations.
Demonstrated appropriate interpersonal skills, active listening, empathy, social awareness, and emotional intelligence in verbal communications
Excellent organisational, time management skills, with the ability to be self-directed and work independently
Demonstrated proficiency in using various computer programs including Microsoft Office, Outlook, One Note, and client record management systems
Demonstrated ability to record interactions and information in a client record management system
