Job Duties

We are seeking a highly motivated Community Health Coordinator to join the Chickahominy Health District’s Community Health Improvement, Preparedness, and Response Team with a homebase in Ashland, VA as well as regular travel to all the counties in the district.

The Community Health Coordinator (CHC) position will develop and facilitate the workplans for the Community Health Assessments (CHAs) and Community Health Improvement Plans (CHIPs) that take place on a continuous cycle in all four counties the Chickahominy Health District (CHD) serves. This position will coordinate trainings and provide information to the public and health district staff on various health topics including social determinants of health, Public Health 3.0, and population health initiatives. The CHC will represent the health district at community events and meetings, plan and facilitate workgroups and steering committee meetings, and partner with CHD staff as well as various organizations, including schools, care providers, businesses, local government agencies, non-profit organizations, and many others, to improve community health. The CHC will support programs and services that are identified through community health assessments, including but not limited to: chronic disease management, mental health care access, maternal and infant health initiatives, transportation options, housing gaps, food insecurity, rural health needs, and harm reduction services. The incumbent will also review, analyze, and present community health indicators to community members and partners to help maintain the district’s position as the chief health strategist.

Core Duties:
1. Performance Management:
• Serves in a supervisory capacity and conducts performance management activities for staff.
• Establishes and reviews work assignments and priorities, and resolves related supervisory issues, in conjunction with program management and Human Resource staff.
• Ensures relevant training and workplace safety for staff and conducts appropriate performance monitoring and appraisal.

2. Health Education & Promotion:
• Obtain population health-related data regarding community demographics, social and cultural environments, growth and development factors, needs and resources; determines need for community health education.
• Plan, implement and evaluate community-based population health improvement programs and/or policies.
• Provides guidance and education to staff and internal/external customers regarding chronic disease prevention and wellness promotion.
• Identify, implement and evaluate opportunities to improve health; includes environmental and policy changes; formation of health promotion programs and opportunities.
• Coordinates and schedules activities with groups interested in making communities healthier.
• Develop/support alliances to broaden the response to health inequities; includes attending/conducting meetings and/or training sessions.
• May utilize or direct the use of social media to inform the community of population health programs.
• May develop seminars, learning sessions or other community trainings; enhances health department’s role as an expert body in public health.

3. Resource Management & Documentation Review:
• Assures health department records are secure and follows HIPAA regulations.
• Prepare grant applications (as appropriate); coordinate grant activities; prepare reports.
• Actively seeks funding; monitors budget and expenses; ensures resources are appropriately and efficiently used.

4. Quality Improvement & Collaboration:
• Writes, edits and designs CHA/CHIP reports; submits reports on goals, objectives, deliverables, and outcomes for community health projects/services.
• Verifies data for accuracy and identifies trends, develops projections and prepares reports/summaries as directed or follows established procedures.
• Supports supervisor as a liaison for local health district and partner agencies on population health matters.
• Promotes integration, consistency and collaboration.
• Collaborates with partnering agencies to develop and deliver community health improvement opportunities.
• May be required to perform other duties; may include functions in support of an emergency declaration.

Minimum Qualifications

· Knowledge of and ability to: collaborate with a diverse group of community partners; experience planning, implementing, and evaluating public health programs; excellent oral and written communication skills; proven ability to work independently

· Ability to use Microsoft Word, Excel, PowerPoint; Ability to manage social media platforms; Ability to integrate knowledge of socioeconomic and behavioral determinants of health in chronic disease prevention work

· Knowledge of CHA/CHIP activities; experience collecting, analyzing, and presenting data to drive activities and prioritize needs; experience managing a program budget; experience integrating principles of cultural sensitivity and appropriateness in chronic disease-related health promotion work; experience in grant writing and management; experience in teaching chronic disease prevention/ health promotion; experience utilizing evidence-based strategies to improve population health; and experience utilizing policy, systems, and environmental strategies
