Job Duties

The Virginia Department of Health (VDH) is dedicated to protecting and promoting the health of Virginians. The VDH is made up of a statewide Central Office in Richmond and 33 local health districts. The Chesterfield Health District provides public health services to the residents of Chesterfield County, Powhatan County, and the City of Colonial Heights. This position is based in Chesterfield.

The Chesterfield Health District (CHD) is seeking a Community Health Worker (CHW) to support patient and client care resource coordination, health education, community engagement, and interpreting Spanish/English within VDH programs. This position is rooted in health equity by bridging individual community members to available social-care programs, improving health access for our most vulnerable residents.

Specifically, this position will: work 1:1 with clients and patients to determine what needs exist and facilitate referrals and connections to programs; deliver education on relevant topics to small groups of constituents; and train to empower residents as community champions of health. This position engages with community members and represents the health district at partner locations, and community events. The position works on a team to for program identification, problem solving, and schedule to ensure scheduled service needs are met.

The Community Health Worker will have a rotating work site at health departments, and regular travel within the Health District is required to partner sites. Typical work hours will be Monday-Friday, 8:00 AM – 4:30 PM, with occasional evenings and weekend activities, as scheduled on a team.

Minimum Qualifications

• Knowledge of common community health problems; public/private health care
practices; community organizing, survey methods and use of data and health
education principles

• Skills in program planning, implementations and evaluation; community
assessment and organization; writing reports and program plans; public
speaking; managing group dynamics and providing consultation with other
professionals and the general public

• Ability to communicate effectively orally and in writing; utilize excellent
interpersonal skills; maintain good working relationships; work independently
with minimal supervision; execute problem solving skills; make appropriate
decisions and develop program goals and objectives; evaluate program
outcomes, impacts and processes

Additional Considerations

• Bilingual in English/Spanish
• Medical interpreting certificate
• Demonstrated experience in care resource coordination, or health navigation.
• Demonstrates ability to work with culturally diverse persons and communities, with the ability to be culturally sensitive and appropriate.
• Demonstrated experience using including Unite Us or FindHelp software.
• Demonstrated experience in delivery of evidence-based, health education curriculums
