Job Duties
Under the direction and supervision of the Benefits Coordinator, the Social Work Discharge Planners provide a range of casework activities related to a comprehensive needs assessment involving each patient’s discharge planning process. The Social Work Discharge Planners will provide an array of services including discharge planning, social work services on the Units and assistance to the Benefits Coordinator with applications. This position will consistently demonstrate a positive and proactive manner of approaching tasks and working with colleagues and external stakeholders.
Minimum Qualifications
* Ability to work in a secure and locked environment with individuals who are experiencing acute symptoms of mental illness and/or additional forensic/legal involvement.
* Working knowledge of social work casework, human behavior and development, how emotional problems can negatively affect individuals and families, and of available community resources.
* Demonstrates ability to form a working relationship with patients and families; determine and select services; and
* Ability to communicate effectively, orally and in writing.
Additional Considerations
* Experience in an inpatient psychiatric, healthcare facility, acute care, long-term care or correctional setting is preferred.
* Experience in case management, discharge planning, and collaboratively working with various community social service and behavioral health agencies.
* Experience and knowledge of benefits/application processes considered.
* Associates degree or higher in human services, social work or related field considered.
