Job Duties
A. Programming (Adult/Geriatric)
• Demonstrate knowledge, skills, and ability to provide Physical Therapy programming, evaluations, and treatments for adult and geriatric individuals. · ·
• Collaborates with others involved in care to facilitate increased functional patient outcomes ·
• Within 5 working days of receiving consult, evaluate and document treatment needs in EHR. ·
• Within 3 days of evaluation, develop/implement Physical Therapy treatment plan. ·
• Develop and implement treatment protocols with clear objectives to meet the needs of patients. ·
• Communicate pertinent information and concerns to the treatment teams, physicians, and nursing staff.
• Redirect patient behavior, as needed.
• Maintain a safe and hazard-free therapeutic environment .
• Report accurate daily feedback on changes in patient behavior to the treatment team and PSR Director.
B. Maintain Quality Documentation
• Follow department documentation guidelines and time frames with at least 95% compliance.
• Document after completing assessments on both the PSR initial assessment sheet and the significant findings form.
• Complete interdisciplinary notes as needed related to behavioral changes, as well as patient events (e.g. Pink hospital event form)
• Complete timely and accurate documentation using Electronic Health record.
• Document any reportable incidents on event report form and submit to proper supervisor.
C. Patient/Family Education and Staff Education
• Coordinate and facilitate PT related and Physical Rehab sponsored educational sessions and in-services.
• Assist with the development of educational material.
• Collaborate with and educate treatment teams, nursing, and support staff to ensure carryover and maintenance of functional gains
General Responsibilities
• Meets with Supervisor and/or Department Head, as scheduled, to communicate needs for assistance, supplies or equipment and receive feedback.
• Works cooperatively with other PSR and hospital staff as demonstrated during supervisor’s observation, and without complaints from departmental/interdisciplinary team members.
• Arrives at work site daily in a timely manner with minimal unscheduled absences.
• Other duties as assigned.
Minimum Qualifications
* Knowledge of principles and practices of recreational/physical therapy; ability to assess, plan, implement, evaluate, and document related to programs for individuals referred to groups.
* Ability to collaborate with peer therapists, interdisciplinary treatment teams, and demonstrate positive communication skills
Additional Considerations
Previous experience in a Psychiatric facility.
