Job Duties
Crater Health District is seeking a dynamic, passionate, and dedicated individual to join our Overdose Prevention Team as a Community Linkages of Care Coordinator. This position is funded by the OD2A-S grant for a 4-year period, ending in August 2028, with funding availability to be reviewed periodically. Continued employment is contingent on the ongoing availability of funds.

As a Community Linkages of Care Coordinator, you will play an essential role in tackling the substance use epidemic in our community. Your responsibilities will include:
Develop case management system under the guidance of the Overdose Prevention Program Coordinator and provide case management for individuals with substance use disorder. Updating response plans to address substance use, establish and maintain partnerships that provide linkage of care activities for individuals, and engage in strategic planning activities in partnership with VDH, state and local Drug Overdose Prevention Program Coordinators. A special emphasis will be placed on health equity, and providing training, outreach and higher-level community technical assistance to non-traditional multi-sector groups not yet addressing substance use. Priority populations include marginalized groups, people who use and/or inject drugs, homelessness. This position will report on grant funding strategies of the Overdose to Action (OD2A) grant. This position will not supervise staff.
This position is situated within the esteemed Epidemiology Program, a key initiative dedicated to communicable disease prevention, public education, and overdose prevention efforts. Our program, along with the district, takes immense pride in delivering exceptional customer service to both community members and partner organizations. This position requires travel within the district as needed, including to the cities of Hopewell, Emporia, and Petersburg, as well as the counties of Sussex, Greensville, Surry, Prince George, and Dinwiddie.

Minimum Qualifications
• Working knowledge of evidence-based Drug Overdose Prevention Principles
• Considerable experience with case management
• Experience with database management systems
• Ability to conduct accurate data entry
• Strong organizational skills with the ability to manage multiple projects
• Experience working with diverse groups
• Demonstrated effective oral and written communication (Microsoft Suites, Email) with the ability to communicate effectively and collaboratively with clients, staff, colleagues, community groups and the general public
Additional Considerations
• Certified Health Educator Specialist, Peer Recovery Specialist, Community Health Worker Certification or Project Management Professional Certification is preferred.
