HWDMC is a 94-bed medical facility that provides acute medical diagnosis, skilled, intermediate and long term care to individuals receiving services in facilities operated by the Commonwealth of Virginia’s Department of Behavioral Health and Developmental Services, as well as individuals with intellectual and/or developmental disabilities within the community. We have been awarded the Joint Commission’s Gold Seal of Approval and Centers for Medicare and Medicaid Services (CMS) 5-Star Rating. Hiram W. Davis Medical Center is currently recruiting an Admissions and Discharge Manager RN to provide management of the admissions process to all levels of inpatient care (General Medical, Nursing Facility, and Skilled Nursing) and the discharge process for individuals with ID/DD discharging to community placements. The Admissions and Discharge Manager provides oversight and training of the admissions and discharge processes with the LPN Admissions and Discharge Coordinators while assisting with these caseloads as well. This position provides marketing to other DBHDS facilities and external stakeholders (hospitals, CSBs, families, guardianship programs, and private providers). The manager must communicate effectively within the facility to interdisciplinary team members as well as with external stakeholders.
Job Duties:
- Performance management of employees supervised
- Responsible for directing, coordinating, and integrating all Admissions & Discharge department services
- Provides direct/indirect administrative and clinical guidance, direction, and leadership to the Admission & Discharge Coordinators (LPNs).
- Manages and monitors staff’s job performance through established administrative and clinical processes and gives feedback.
- Responsible for planning and recruiting an adequate number of personnel to fulfill the service delivery needs of the department.
- Responsible for oversight of fund expenditures for departmental budgets.
- Responsible for establishing, annually reviewing, and revising as needed all policy and procedures to ensure compliance.
- Assures integration of services by establishing collaborative practice with Medical Staff, Nursing, Ancillary, and Administrative services. Establishes good working relationship and fosters good communication among integrated services.
- Familiar with requirements/criteria for all Nursing Home levels of care at HWDMC – Nursing Facility and Skilled Nursing Facility.
- Educates external stakeholders in services HWMDC provides and process for admission.
- Collects and reviews pertinent information to assist in the decision of appropriateness of admission. Provides information to Medical Director and seeks approval. Consults with the Chief Nurse Executive as needed regarding appropriateness or needs of an admission.
- Screens admissions for complications beyond the capacity of HWDMC. Seeks approval from the Facility Director before denying admission to the facility.
- Coordinates admission through facilitating communication between the discharging facility and HWDMC, disseminating information to the HWDMC interdisciplinary team, facilitating pre-admission meetings as needed, and coordinating with Forensic/Security staff of other DBHDS facilities regarding the legal aspects of admission as needed.
- Schedules admissions when possible. This is done by balancing the needs of the facility, the community, the resident/patient, and the physician.
- Coordinates and leads discharge process to the community for Nursing Home ID/DD residents with all team members, CSB, AR/LG, residential provider, Community Integration Manager, and other community providers as needed.
- Coordinates team meetings, and works with providers, CSBs, families, ARs/LGs, Durable Medical Equipment (DME) vendors, and pharmacies to ensure supports are in place before discharge.
- Coordinates training needs for discharge as needed.
- Conducts Post Move Monitoring for the ID/DD discharge process to community placements.
- Manages documentation for this process including discharge planning, training, visitation, and post move monitoring documents as well as prescriptions, orders, and letters of need.
- Provides consultation to HDMC Social Workers upon request re: the medical needs of residents and patients on their caseloads needed for discharge.
- Participates in weekly Discharge Update Meeting reviewing SNF/NF residents with ID/DD who will discharge to the community with CIM, Clinical Director, and Social Work Department.

Minimum Qualifications
- A valid Virginia RN license
- Clinical experience in a Hospital and Nursing Facility.

This position requires excellent oral and written communication skills and requires someone with a positive attitude, who is friendly and comforting and must be able to interact with others with ease. This position must be able to work interactively with all clinical disciplines and establish effective interpersonal relationships. Computer skills, including comfort with using Microsoft Office Suite, are required. Close attention to detail and good organizational skills are required.
Additional Considerations
Experience in nursing at the professional level coupled with experience in Admissions & Discharge nursing, psychiatric nursing, geriatric nursing, rehabilitative nursing and or nursing care of the intellectual disabled.
