HWDMC is a 94-bed medical facility that provides acute medical diagnosis, skilled, intermediate and long term care to individuals receiving services in facilities operated by the Commonwealth of Virginia’s Department of Behavioral Health and Developmental Services, as well as individuals with intellectual and/or developmental disabilities within the community. We have been awarded the Joint Commission’s Gold Seal of Approval and Centers for Medicare and Medicaid Services (CMS) 5-Star Rating. Hiram W. Davis Medical Center is currently recruiting Licensed Practical Nurses to be responsible for performing resident skin assessments, obtaining treatment orders, administering treatments as ordered, and documenting appropriately to help ensure that resident skin integrity is improved and/or maintained.
Job Duties:
- Calls physicians regarding treatment orders.Takes verbal and telephone orders from physician. Transcribes physicians' orders under the supervision of a registered nurse
- Stays abreast of facility operations by reading e-mails. Keeps computer access current. Stays abreast of resident/patient care information by participating in shift-to-shift reports and making rounds.
- Collaborates effectively with OT, PT and other disciplines in the care of pressure wounds.
- Collaborate with all care- givers and obtain feedback from RNs, LPN’s and C.N. A’s who are familiar with the care and assessment of patients.
- Act as a liaison to the wound consultant.
• Administers treatments as prescribed within standard nursing practice guidelines and in full compliance with established infection control protocols.
• Initials and dates dressings to ensure that changes are done timely as prescribed.
• Conducts and / or leads weekly wound rounds to assess resident skin condition. Documents findings appropriately and contacts physician to make recommendations for new or revised treatment orders.
• Receives, transcribes and implements new treatment orders in a timely manner.
• When requested by immediate supervisor or others in nursing leadership, evaluates a resident’s skin or make arrangement for evaluation.
• Monitors the progress of wound healing and makes recommendations for changes as appropriate.
• Calculates weekly / monthly pressure ulcer rates.
• Performs and records weekly pressure ulcer assessment and measurement documentation
Minimum Qualifications
- Current licensure as Licensed Practical Nurse with the Virginia State Board of Nursing.
- Graduation from an accredited School of Practical Nursing.
- Experience in nursing at the licensed practical nurse level in a long-term care setting with experience in wound care, or medical/surgical nursing with wound care experience.
KSA's required to successfully perform the job:
Clinical knowledge of principles and practices of practical nursing and ability to implement planned nursing care and provide guidance to other staff. Knowledge of normal anatomy and physiology, pharmacology, body mechanics, nursing procedures, growth and development, and cardiopulmonary resuscitation. Skills in performing patient assessments, to observe, record and evaluate patient’s response to treatment, to communicate effectively with others orally and in writing.
Additional Considerations
- Working experience as a Licensed Practical Nurse within the field of medical-surgical, psychiatric, rehabilitation, geriatrics or intellectually disabled preferred.
- Certification in Wound Care preferred
