Job Duties
Code selection and data entry are performed accurately and in a timely Fashion. Admission Summaries and Discharge Summaries are coded and data entered for in house indexes. Liaisons with other clinical departments to ensure accurate and timely records are maintained in compliance with documented disease coding standards. Provides coding attestations to reimbursement and UR departments to aid in billing functions.
Performs admission and discharge audits to meet hospital guidelines and department goals. Follows established audit guidelines and report findings to HIM Director by the 15th of the following month. Performs Core Measure audits for all admission, discharge and transitional records; ensures entry of result findings in Avatar by the 15th of the following month. Provides results of monthly and/or quarterly audit reviews to HIM Director, and Director of Quality and Risk Management as requested.
Provides HIM Director with a list of incomplete admission histories, discharge summaries, and other missing documentation for reporting purposes. Provides clinical documentation training to staff as required.
Prior to admission reviews the patient’s admission packet, determining if the patient is new or a readmission. Promptly answers call on patient’s arrival. Interviews patient for information to complete admission documents. Completes, copies and distributes all admission documents per established guidelines. Keys admission patient data accurately into AVATAR and HOPs databases. Scans/Imports Facesheet into the EHR in timely manner for unit. Pulls previous records for admission unit as necessary.
Assists with census collections and reconciliation in coworker’s absence. Provides assistance with preparation with commitment hearings.
Review scanned documents for accuracy, clarity, appropriate naming and folder selection. Monitors for potential HIPAA, and filling issues. Reports issues to the HIM director, corporate compliance and the unit secretary.
Minimum Qualifications
Knowledge, Skills, Abilities (KSA) and or Competencies required to successfully perform the work (attach Competency Model, if applicable): Significant knowledge of HIM practices and procedures; ability to review clinical records for completeness, accuracy, and compliance with regulations; experience with medical and classification software; proficiency compiling clinical data and writing statistical reports; strong knowledge and experience in administrative systems and records management; knowledge of The Joint Commission and CMS regulations; Demonstrated knowledge applying coding principles.
