Job Duties

The Director of Social Services provides administrative and operational leadership to the Social Services department by planning, directing, and coordinating the activities of the social services programs of the facility and providing for the needs of our resident Veterans and to fully enhance their quality of life. The Director of Social Services is responsible for the organization, supervision, and administration of social services staff to include planning, directing of workforce, controlling costs, and maintaining organizational relationships. The Director of Social Services is responsible for developing, implementing, and managing a comprehensive social work program to meet the needs of our residents and their families.

Job Requirements:
• Plans, coordinates, executes, and supervises all daily activities for the Social Services Department.
• Plans, directs, and coordinates comprehensive clinical social work services to include but not limited to identifying and evaluating personal, emotional, and environmental concerns of residents, counseling residents in individual and/or group settings, and monitor, evaluate, and record resident progress according to measurable goals as described in their treatment care plan.
• Work with the IDCP team to develop a care plan individually designed to assist the resident in attaining the highest practicable physical, mental, and psycho-social functioning.
• Complete Resident Full Assessment Form and Progress Notes by the Assessment Reference Date (ARD) and place on medical chart.
• Completes MDS assessments, CAAs, develops and updates care plans as required/needed.
• Functions as the facilities grievance officer and handles, distributes, follow up on customer concerns and grievances.
• Oversees and coordinates visits with the podiatrist, dentist, optometrist, and other specialists that come to the facility.
• Assists with the PASSAR process
• Oversee and manage the barber/beauty salon staff and supplies.
• Provide staff with frequent, constructive feedback to include but not limited to performing new hire, mid-year, and year end employee evaluations and in-service training as needed or required.
• Determine staffing requirements, and interview, hire, and train new employees or oversee these processes to ensure successful staffing.
• Create long-term work schedules to maximize departmental outcomes utilizing the appropriate levels of staffing and assigning specific duties.
• Establish and implement goals, objectives, operational standards, and procedures for the department.
• Monitor and assesses performance of staff to make improvements or take corrective action as needed.
• Coordinate activities with other departments to ensure that services are provided in an efficient and timely manner.

Minimum Qualifications

• Bachelor’s degree in social services, social work, or related field.
• 2 Years prior social work experience in a Long-Term Care (LTC) facility.
• Considerable knowledge of principles and methods for providing supervision to include the ability to instruct and develop staff and ability to plan and direct the work of subordinate staff.
• Considerable knowledge of principles and methods for provision of social services including but not limited to advocating and providing direct support to residents and their families, providing necessary referrals to aid residents and their families in obtaining proper care, monitoring, evaluating, and documenting resident progress.
• Working knowledge of the theory, techniques, and practices of counseling, social work case management, conflict management, and mediation.
• Demonstrated clinical knowledge regarding services which may be provided in a skilled nursing care/rehabilitation facility.
• Knowledge of business and management principles involved in strategic planning, resource allocation, human resources modeling, leadership technique, production methods, and coordination of people and resources.
• Possess basic knowledge of computer software such as Microsoft Office.
• Ability to effectively communicate both written and orally.

Additional Considerations

• A Master’s degree in Social Work is preferred.
