Minimum Qualifications
Key Responsibilities
• Coordinate all phases of nutrition care including nutrition assessment, resident nutritional interviews, care planning, monitoring, education, and counseling of resident nutritional intake
• Provide nutrition services and medical nutrition therapy integrated with the resident’s medical goals
• Evaluate, interpret, monitor, and document the nutritional status and nutritional needs of residents using established standards of care and practice guidelines
• Develop, review, update, and implement educational materials to meet the nutritional needs of residents
• Function as an integral member of the care plan team and attend Quality Improvement meetings
Additional Considerations
What We’re Looking For
•Prior experience with therapeutic or medically necessary menus in healthcare or long-term care settings
•Experience with altered diets, including educating staff on IDDSI standards and testing meals for consistency
•Strong attention to detail and accuracy in all tasks
•Excellent communication skills, both verbal and written
•Prior experience performing sanitation audits in healthcare kitchens
•Proficiency in Microsoft Office Suite (Excel, Word, Outlook)
•Experience with documenting in EMR Systems (Point Click Care preferred) for State and Federal regulations
