Position Overview:
Western State Hospital is seeking a full time Patient Care Investigator to serve as the part of the facility’s investigation team to investigate alleged abuse/neglect, peer to peer and administrative reviews as assigned by the Director, including dLCV reported incidents.
Key Responsibilities:
- Provide the Operations/Quality Improvement Office investigative reports, attachments, various recorded evidence related to an investigation; monthly case status review reports; and maintain communication with the Investigations Manager, Facility Director and the Human Rights Advocate
- Represent each investigation during grievances and other administrative settings; unemployment hearings, etc. when needed
- Coordinate the investigative process with the DBHDS Human Rights Advocate and the Department of Social Services and conduct other administrative investigations as instructed by the director
- Maintain database for ongoing investigations as and when received, extension requests and completion date of assigned investigation
- Meet with Director on a regular basis to keep Director up to date with investigations/issues, etc.

Minimum Qualifications
- Combination of training/experience in patient investigation as a patient investigation specialist and experience with strong report writing
- Mandated investigation training by DBHDS and be on an approved list of investigators
- Good investigative techniques, organizational skills, writing and interviewing skills
- Ability to communicate effectively, both orally and in written form with a variety of individuals from a variety of professional backgrounds
- Individual must have the ability to write, clearly, concisely and in an extremely organized manner
- High degree of independent initiative and decision-making
- Must be able to function independently, assess and plan an effective investigation
- Must possess moderate skill in the use of computers.
