Job Duties

Evaluation and treatment planning: 30%
Comprehensive assessments, social work reassessments, treatment plans, and other required medical record documentation are completed in accordance with departmental policies and procedures, accrediting/certifying body regulations, and state/federal laws as evidenced by medical record reviews, supervisor/peer reviews and results of accrediting/certifying body reviews.

Assesses patient's discharge needs and coordinates the discharge plans with the CSB: 30%
Initiated upon admission and across the continuum of care discharge planning is provided in an effective and efficient manner according to policy and DBHDS discharge protocols.
Discharge needs are clearly documented and plans for patients recommended for discharge are updated in weekly notes and ready for discharge form in the EHR.
Notes identify barriers to discharge as well as progress made to alleviate barrier.
Discharge plans and referral summaries are clearly written and describe a comprehensive discharge plan.

Provides individual, family and group counseling and patient/family education: 20%
Documentation indicates content of individual, family and group sessions and provision of education to patients and families. It is completed according to departmental policy.

Treatment planning: 15%
Participates with the interdisciplinary team in formulating individual treatment plans.
Functions as plan author of assigned patients.
Treatment plans are completed thoroughly, patient-centered, and timely according to facility guidelines and governing bodies.

Additional duties: 5%
Provides admission on call services on a rotating basis.
Communicates with nursing supervisor, CSB, other facilities, Central Office, Jail and others to assist with admission needs. Consults with patient flow manager and/or social work director regarding difficult admissions.

Minimum Qualifications

Bachelor's degree in Social Work (BSW) from an accredited college or university.
Knowledge of population specific factors in assessment and treatment. Knowledge of theories of human behavior and development. Skills and ability in providing individual, family and group counseling. Knowledge of community resources. Advanced interviewing skills. Knowledge of Social Work Code of Ethics. Ability to make independent work decisions. Ability to independently maintain clinical records on assigned cases in accordance with hospital policies and governing body regulations.

Additional Considerations

MSW, LCSW highly desirable; experience in behavioral health and/or working with a geriatric population is also desired.
