Job Duties
To assist the team in preparing residents for conditional release. To plan, organize and implement a range of services to residents, which will promote recovery, community placement and reduce possibilities of readmission. Assist in the coordination of discharges. Coordinate all transportation needs to include completing all forms and reserving vehicles. Coordinate community passes and assist residents with job applications, trips to learn about environment they will reside in and assist residents learning transit services. Search for resources for residents to assist the Social Workers with discharge needs. This position will also assist with program development and performance improvement as needed. Document all contact and behaviors of residents.
Minimum Qualifications
• Knowledge in social work principles, services, techniques and practices; social histories and assessments, case management methodologies (i.e. coordinating resources for the patient).
• Ability to interact with other professionals as a contributing team member
• Knowledge of Social Security, Medicaid, and Medicare application processes and eligibility criteria is preferred
• Working knowledge of available social, economic and community resources
• The ability to assist in coordinating resources for residents; ability to interview and gather pertinent information from patients and family
• Ability to perform at a fast pace with frequent changes.
• Experience with sex offenders is preferred but not essential.
• Ability to, in conjunction with interdisciplinary teams, develop goals and implement treatment plans.
• Ability to evaluate and document treatment progress; exercise independent judgment in coordination and delivery of services and collaborate with treatment team, conflict management, and mediation
• Demonstrated ability and skills in providing task orientation and facilitate psycho educational groups
• Knowledge, skills and ability to communicate effectively, orally and in writing (basic word processing and computer skills)
• Ability to use good judgment
• Ability to work evenings, weekends and overtime
Additional Considerations
• Experience working with residents in a forensic environment preferred.
