Job Duties

Performs accurate and timely medical record coding, data entry, and documentation audits for admissions, discharges, and clinical records in accordance with established hospital and disease coding standards. Reviews patient admission information, completes and processes required admission documentation, enters patient data into applicable systems, and ensures medical records are accurately maintained and scanned into the electronic health record. Identifies incomplete or missing documentation, provides clinical documentation training as needed, supports census reconciliation and commitment hearing processes, and reviews scanned documents for accuracy, appropriate filing, and compliance with HIPAA requirements.

Minimum Qualifications

Knowledge, Skills, Abilities (KSA) and or Competencies required to successfully perform the work (attach Competency Model, if applicable): Significant knowledge of HIM practices and procedures; ability to review clinical records for completeness, accuracy, and compliance with regulations; experience with medical and classification software; proficiency compiling clinical data and writing statistical reports; strong knowledge and experience in administrative systems and records management; knowledge of The Joint Commission and CMS regulations; Demonstrated knowledge applying coding principles.

Additional Considerations

HIPAA Access Levels: 1 Level One: Complete Access to all client/patient related Protected Health Information (PHI.)

Experience in Medical Coding or EHR is highly desired.
