Purpose of Position

The purpose of the position is to coordinate the resource facilitation program for the North Dakota Brain Injury Network. NDBIN provides support, education, and outreach regarding brain injury across North Dakota. The position will guide and support resource facilitators statewide including providing mentorship, feedback, and day-to-day guidance to ensure quality, consistent service delivery. Position will work closely with the program director, senior project coordinator, resource facilitators, and administrative assistant(s); as well as directly with state, regional and national partners.

Duties & Responsibilities

Provide leadership and oversight for the NDBIN resource facilitation program.

  • Oversee NDBIN main line (receive and return calls, refer calls to resource facilitators as needed, return voicemails, maintain outbound messaging).
  • Manage the NDBIN referral process including receiving, reviewing, and assigning to appropriate Resource Facilitator.
  • Guide and support resource facilitators statewide including providing mentorship, feedback, and day-to-day guidance to ensure quality, consistent service delivery.
  • Coordinate resource facilitation team meetings and help resolve operational challenges.
  • Oversee resource facilitation cases including supporting team members in tracking progress toward individual and team goals, serve as a resource for process questions & referral coordination, and assist with performance tracking and reporting.
  • Help onboard new staff and support their professional growth and integration into the program.

Provide resource facilitation services.

  • Maintain a direct service caseload of individuals with brain injury and their families.
  • Provide person-centered resource coordination and problem-solving support.
  • Conduct intake, ongoing needs assessments, and follow-up planning.
  • Navigate and help individuals access medical, vocational, educational, and other supports.
  • Track outcomes, document services, and contribute to program evaluation.

Lead resource facilitation outreach and education.

  • Oversee resource facilitation outreach and education to communities.
  • Establish and maintain working relationships with various facilities, organizations and agencies to facilitate, establish, and expand program referrals.
  • Support NDBIN’s education and awareness campaigns.
  • Participate in statewide outreach efforts, track on resources that are found in communities across the state and assist with regional resource mapping.
  • Maintain updated listing of state support groups and serve as a supportive liaison for regional support groups.
  • Assist in the organization and hosting community outreach and peer support events.
  • Assist with the creation and dissemination of brain injury educational materials.

Assist with program development, evaluation and support.

  • Attend all program and staff meetings to ensure continuity among program staff.
  • Attend the annual mind matters conference and other professional development opportunities.
  • Participate in program development discussions based on existing data at least annually.
  • Collaborate to develop additional data analysis strategies as needed.
  • Compile any required periodic reporting as requested by program director or senior project coordinator.
  • Respond by phone, email, and in a professional manner.
  • Represent the program at meetings, conferences, seminars, and workshops as needed.
  • Assist with meeting registration, setup, and logistics. 
  • Other related duties or special projects, as assigned.

Required Competencies

  • Knowledge of community, state, and federal resources for brain injury, disability, or human services.
  • Knowledge of home and community-based service system and Medicaid.
  • Ability to connect with individuals with resources, empower interested persons or organizations, and take on leadership roles.
  • Ability to accurately type, proof, edit, and format reports and manuscripts, create agendas, record minutes.
  • Customer service skills.
  • Experience participating in large and small group meetings, in person and by video-conferencing.
  • Strong computer aptitude. Ability to troubleshoot IT issues.
  • Skills and ability using the internet and search engines.
  • Experience, skills, ability in organizing and tracking programmatic activities.
  • Strong problem-solving and decision-making skills.
  • Excellent interpersonal, verbal, and written communications skills.
  • Demonstrated organizational skills and ability to be self-directed.
  • High level of attention-to-detail and the ability to generate timely work products.
  • Ability to prioritize, multi-task, take initiative, and to collaborate effectively as part of a team.
  • Must be able to travel to national, regional, or local events/meetings.

Minimum Requirements

  • Bachelor degree in human services or behavioral health field of study.
  • Five years’ experience working in home health, social services, or working with individuals with disabilities.
  • Three years’ experience in brain injury resource facilitation.
  • Experience using videoconferencing platforms such as Zoom, WebEx (or similar).
  • Successful completion of a criminal history records check.

Preferred Qualifications

  • Degree in Social Work, Psychology, Sociology or related emphasis.
  • Experience working in or with North Dakota social service, human services, or healthcare systems.
  • Experience supervising staff.
  • Experience organizing, tracking, and writing programmatic reports.
  • Experience providing phone-based support and technical assistance to individuals.
  • Experience facilitating meetings and presenting.
  • Experience working with people with cognitive disabilities and/or behavioral health challenges.
