The Opportunity:

This role is initially offered on a two-year fixed-term contract, with the possibility of extension or transition to a permanent position.

Reporting to the Home Care Manager:

  • You must be tech-savvy and quick to learn new systems
  • You will provide coordination services to support our consumers. You will be a key team member helping to deliver a variety of services to meet out consumers’ unique circumstances and needs, ranging from entry level through to complex health needs.
  • You will oversee a portfolio of consumers, develop and manage care plans, as well as create services and supports which are unique to individuals.
  • Administratively, you will be responsible for a range of tasks such as drafting budgets, monitoring financial transactions, actioning changes in care, undertaking general marketing, as well as working with staff to deliver exceptional services.
  • You will liaise with Medical Professionals and Allied Health Services with an integrated approach to consumer care
  • You will ensure compliance with the Aged Care Quality Standards at all times
  • You will actively promote the growth of UPA Home Care services in the Western NSW region

What you bring:

  • Certificate IV in Aged Care or Diploma or Degree in Community Services,
  • Demonstrated experience in home care packaging, case management, assessment, care planning and service coordination
  • Understanding of consumer directed, community-based care service delivery
  • Understanding of current Aged Care Legislation, Aged Care Principles and Aged Care Standards
  • First Aid and CPR Certificate (desirable)
  • Experience working with and adhering to budgets
  • Ability to liaise with and promote our services to external providers in a proactive and positive manner
  • Computer literate (Word, Excel, email, internet)
  • Current Driving License
  • Demonstrated strong interpersonal skills
  • Exceptional customer service skills, with both internal and external customers
  • Applicants must have permanent residency or be an Australian Citizen
  • Applicants must be willing to undertake a Police Check prior to commencement

Pre-employment screening:

The successful candidate is required to have completed/have a willingness to undertake the following:

  • Vaccinated in accordance with UPA’s Immunisation and Vaccination Policy. (including proof of the required doses of a TGA approved COVID-19 vaccine and annual influenza) 
  • Undertake a National Criminal History Check 
  • Pre-Employment Medical Assessment 

Appointment is subject to satisfactory of all pre-employment checks.
