At Jewish Care Victoria, you’ll be part of our Active Living Services team supporting older adults through a person-centred, evidence-based approach that prioritises continuity of care and long-term outcomes.

This role offers variety, autonomy and the chance to truly connect with your clients – helping them stay active and independent in the community they love.

What you will do:

  • Initial assessment of clients and development of individualised care plans
  • Perform client reviews using appropriate outcome measures
  • Promote independence and functional abilities for both residential and community clients, with a strong focus on health promotion
  • Communicate client outcomes at relevant clinical/team meetings and documentation in the client data base
  • Promotion and understanding of cultural diversity in care services

You will have:

  • Degree in Occupational Therapy
  • Registration with the Australian Health Practitioners Regulation Agency (AHPRA)
  • Demonstrated knowledge of a Wellness Reablement approach to service delivery in community aged care
  • Experience in the development, facilitation and evaluation of group programs is desirable, but not essential
  • Proven clinical experience in community aged care or a related field
