The Role
We are looking for an experienced Care Coordinator – Case Management to support the delivery of high-quality, person-centred care across our home and community services. Working closely with Care Partners, clinicians and the wider team, you will:
- Coordinate services to ensure Members receive responsive, person-centred support.
- Support Care Partners with care monitoring, service coordination and case management activities.
- Maintain accurate client records and documentation.
- Liaise with Members, families, clinicians, service providers and internal teams.
- Assist with scheduling, referrals and service changes to meet Members' evolving needs.
- Contribute to quality improvement, compliance and continuous improvement initiatives.
- Support the delivery of services in line with the Aged Care Act 2024 and Benetas policies.
About you
To be successful in this role you will need:
- Experience in care coordination or service delivery within aged care, community services or health.
- A commitment to delivering high-quality, person-centred care.
- Excellent communication and relationship-building skills with Members, families and multidisciplinary teams.
- Strong administration and organisational skills with the ability to manage competing priorities and maintain accurate documentation.
- Sound problem-solving skills and the ability to respond proactively to changing Member needs.
- Confidence using client management systems and Microsoft Office applications.
- An understanding of the Aged Care Act 2024 and Support at Home Program (or the ability to quickly develop this knowledge).
- All persons at Benetas will need to obtain a NDIS Worker Screening Clearance to be eligible for employment or engagement. You can find out more information here.
