Join Our Team as an Aged Care Assessment Team (ACAT) Coordinator

At Limestone Coast Local Health Network, we are committed to delivering exceptional healthcare services across diverse settings. We are currently seeking a dedicated and experienced Aged Care Assessment Team (ACAT) Coordinator to join our dynamic team.

Key Responsibilities:

  • Provide expert nursing and/or midwifery services across diverse health service settings, supported by experience and/or advanced studies.
  • Develop from competent to proficient practitioner, ensuring high standards of practice and mentoring less experienced staff.
  • Support the Nursing/Midwife Unit Manager or equivalent in leading nursing/midwifery teams within the ward/unit/service.
  • Promote continuity and consistency of care in collaboration with AN/MUM colleagues and management.
  • Assist in implementing practice changes and contribute to ward/unit/service management responsibilities including performance management, recruitment, staffing logistics, leave management, rostering, work allocation, and attendance management.
  • Take charge of the development, coordination, implementation, and evaluation of the LCLHN Aged Care Assessment program through acute, residential and community settings.
  • Provide specialist knowledge and skills in the field of aged care assessment, enhancing team capabilities and ensuring excellence in patient care.

About You

  • You must be registered or eligible for registration as a Nurse or Nurse/Midwife with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate with atleast 3 years post registration experience or currently classified as a Clinical Nurse/Midwife
  • You will have extensive experience in aged care service provision, including working with individuals/carers who have dementia or mental health problems and their families
  • Experience in the supervision of student nurses, enrolled nurses and less experienced registered nurses.
  • Experience in management and Leadership roles.
