About the role

The CO-ACT model is a new service which focuses on older adults over 65 years of age who are typically awaiting placement in residential aged care facilities (RACFs) or memory support units and who no longer need care in an acute hospital. This CALHN-run service will support patients from other LHNs by providing short-term placement while their next care facility is being identified, thus addressing barriers to discharge and improving patient flow. Located amongst spacious gardens, Hampstead Rehabilitation Centre provides an environment for these patients which is similar to many residential aged care facilities.

Responsible for the organisation, management and operation of the designated pharmacy area. The provision of a comprehensive high-level range of pharmacy services, to exercise independent and autonomous judgement in the provision of pharmacy services, medicines information and therapeutic advice to patients of the hospital as well as medical and nursing staff and other health professionals in accordance with hospital and divisional policies and procedures and professional standards and guidelines relating to pharmacy practice. The optimisation of outcomes for patients by implementing strategies to achieve the best possible quality use of pharmaceuticals in a safe and cost-effective manner and in accordance with hospital policy.

Essential Requirements:

• Current National Police Clearance (NPC)
• Working With Children Check (DHS)
• Immunisation screening as indicated in the role description. SA Health Services are required to implement the Addressing vaccine preventable disease: Occupational assessment, screening and vaccination policy in the workplace.

Educational/Vocational Qualifications

• Bachelor of Pharmacy or equivalent
• Registered or eligible for registration with the Pharmacy Board of Australia
