About the Role:

This is an exciting opportunity to join the Older Person's Emergency Referral and Assessment team as a Clinical Nurse to make a difference in the lives of older people.

As a Clinical Nurse in the OPERA team, you will play a pivotal role in providing exceptional care and support to older patients. Your responsibilities will include conducting comprehensive assessments, developing tailored care plans, collaborating with multidisciplinary teams, and ensuring a smooth transition of care for patients returning to the community or admitted to hospital.

To be eligible for this position, you must be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and hold, or be eligible to hold, a current practicing certificate.

Lyell McEwin Hospital The major hospital in the north and north-east for emergency care, complex and multi-day surgery, medicine, obstetrics, paediatrics and outpatient services.

Modbury Hospital A general hospital with emergency care, elective surgery, medicine, outpatient and sub-acute services, rehabilitation, geriatric and palliative care.
