About the Role:

As an Occupational Therapist within the Integrated Care team, you will play a vital role in delivering timely, person-centred occupational therapy assessment and interventions, for patients with complex and urgent care needs. Working within a multidisciplinary team, you will support patients at risk of emergency department presentation, hospital admission, or re-admission, enabling safe and independent living at home wherever possible.

You will provide discipline specific assessment and intervention across home, inpatient, and virtual care settings, with a strong focus on function, safety, recovery, and wellbeing. This role involves proactive discharge planning, coordination of supports, and close collaboration with internal and external partners to deliver integrated, high-quality care in line with CALHN values and frameworks.

 Key responsibilities include:

  • Undertaking functional, cognitive, and home safety assessments within urgent care hubs and/or in home settings
  • Providing education and intervention related to falls prevention, energy conservation and risk minimisation.
  • Recommending, organising, and facilitating discharge equipment and home modifications to support safe transitions of care
  • Coordinating referrals and linking consumers with appropriate community, health, and support services
  • Working collaboratively with multidisciplinary team members and external stakeholders to optimise consumer outcomes

About You

You’re a motivated Occupational Therapist ready to support consumers at home and in the community. You bring professional expertise, sound judgement and collaborative skills to provide safe, client-centred care.

  • Hold AHPRA registration as an Occupational Therapist

  • Deliver high-quality care for clients with complex needs

  • Apply clinical knowledge in acute, rehab or community settings

  • Communicate clearly with clients, carers and multidisciplinary teams

  • Work independently while contributing to service improvement
