About the Role
We are seeking a passionate and experienced healthcare professional to provide leadership and coordination in Statewide Care of the Older Person and Community Transition. In this pivotal role, you will oversee the line management of nursing and multidisciplinary team activities, ensuring the seamless delivery of high-quality, person-centred care. Your expertise will drive continuity of care and positive outcomes for patients and clients.
As a key leader, you will be accountable for nursing and multidisciplinary practices, addressing any inconsistencies between policy and practice while fostering a positive and high-performing team culture. Your commitment to excellence will support both staff development and optimal patient outcomes, making a meaningful impact on the lives of older people in our community.
About You
We are looking for a dedicated and experienced Registered Nurse with strong leadership skills and a commitment to high-quality, person-centred care. You will bring a passion for innovation, teamwork, and continuous improvement in the care of older people and community transition.
- Qualifications – Registered or eligible for registration with the Nursing and Midwifery Board of Australia, with a current practising certificate.
- Leadership & Communication – Proven ability to lead teams, resolve conflicts, and drive change, with excellent problem-solving and negotiation skills.
- Experience & Expertise – At least three years of post-registration experience, including leadership, supervision, and the delivery of high-quality nursing care.
- Knowledge & Commitment – Strong understanding of nursing standards, legislation, and contemporary healthcare issues, with a focus on quality and safety in service delivery.
