Seeking a passionate and dedicated Nurse Practitioner or Nurse Practitioner Candidate to join our dynamic team at GEM@HOME
About the role:
As a Nurse Practitioner or Nurse Practitioner Candidate, you will play a pivotal role in providing exceptional care to our patients in the community setting. Your expertise and compassion will be invaluable in delivering high-quality, patient-centered care, focusing on health promotion, disease prevention, and management of acute and chronic conditions.
In this role, you will collaborate closely with our multidisciplinary team, ensuring seamless coordination of care and effective communication with patients, families, and healthcare professionals. Your advanced clinical skills, critical thinking, and decision-making abilities will be essential in assessing, diagnosing, and managing a diverse range of healthcare needs.
About you:
- Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
- Holds Master of Nursing (Nurse Practitioner)
- Demonstrated ability to work as an autonomous practitioner and/or as part of a multidisciplinary team.
