About the Role
RN-3 We are seeking a highly skilled and compassionate nurse with clinical expertise in palliative care to provide direct, person-centred support to patients and families in the aged care setting. In this advanced role, you will offer expert nursing care, lead and coordinate the CPCiAC program, and develop a needs rounding initiative that supports Advance Care Planning and timely palliative responses. With a high level of autonomy and clinical decision-making, you’ll provide leadership, mentorship, and education to aged care staff, helping to build workforce capability and improve resident outcomes. You will also be responsible for reporting on program outcomes and ensuring practice aligns with policy and best standards of care.
RN-4 We are seeking a dedicated and experienced Nurse Practitioner to lead and deliver person-centred palliative care primarily within the aged care setting. This role offers the opportunity to provide expert clinical care, leadership, and education to improve the palliative experience for residents, families, and aged care staff. You’ll coordinate the CPCiAC program, lead needs rounding to support Advance Care Planning, and contribute to sustainable improvements in care outcomes. Working autonomously and within a multidisciplinary team, you’ll operate under an expanded scope of practice, including prescribing, while driving innovation and best practice in palliative and aged care.
