In this position, you will play a vital role in enhancing the care and support provided to older individuals experiencing mental health challenges. As a Carer Consultant, you will work closely with patients, their families and healthcare professionals to ensure a holistic approach to care.
Your responsibilities will include:
- providing expert guidance and support to carers and families navigating the complexities of older persons' mental health
- advocating for the rights and needs of older adults and their carers within the healthcare system
- collaborating with multidisciplinary teams to develop and implement care plans tailored to individual needs
- facilitating education and training programs for healthcare professionals, carers and the community
- promoting positive aging and mental well-being through community outreach initiatives
