About the Opportunity
We currently have an exciting opportunity to join the Cedar Health Service team as a Clinical Nurse, where you will be responsible for:
- Provide proficient, person centred, clinical care and individual case management to consumers in Cedar Health Service: Inclusive Domestic and Family Violence Healthcare.
- Triage consumers, assessing safety and risk of consumers affected by domestic violence. This may include consumers who have experienced strangulation, traumatic brain injury (TBI) and intimate partner abuse
- Monitoring consumer care plans to ensure appropriate care outcomes are achieved at each consumer interaction, whether face to face or over the phone.
- Use knowledge and experience in health assessment, treatment, and implementation to achieve agreed patient care as guided by Clinical Practice Manual and WCHN Procedures. Actively participate in monitoring consumer’s care via multi-disciplinary clinical reviews to ensure quality patient outcomes.
- Plan, implement and evaluate group-based services which contribute to the development of consumer capacity to self-manage their own care.
About You
To be successful in this role, you will have:
- Registered as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
- Registered Nurse with at least 3 years, full time equivalent, post registration experience.
- Excellent communication skills including the ability to communicate effectively with consumers, staff, external agencies, and members of the public, both face to face and over the telephone demonstrating a high level of interpersonal skills.
- Ability to provide specialised domestic and family violence assessment triaging and healthcare such as recording injury’s, which may latter be used in court.
- Respond to the health needs of people affected by domestic and family violence in the health service and ability to provide outreach nursing services in domestic and family violence accommodation.
