About the Opportunity

We currently have an exciting opportunity to join the Metropolitan Youth Health (MYH) team as a Community Liaison Midwife, where you will be responsible for:

  • Deliver client-centred care within a community midwifery model using a case management approach aligned with the MYH Model of Care.
  • Conduct and document comprehensive health services, including assessments, screenings, referrals, treatments, and psychosocial support.
  • Advocate for and facilitate access to primary, acute, and specialist health care services for vulnerable young people.
  • Build and maintain partnerships with clients, interagency teams, and professional networks to support effective health outcomes.
  • Monitor and evaluate clinical service delivery, maintain accurate documentation, and contribute to quality improvement initiatives.

About You

To be successful in this role, you will have:

  • Registered or eligible for registration as a Nurse and/or Midwife by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
  • Registered Midwife with at least 3 years, full time equivalent, post Midwifery registration experience.
  • Well-developed interpersonal and communication skills including the ability to work with individuals and families for positive health outcomes and to foster productive working relationships across disciplines.
  • Demonstrated ability to display initiative, flexibility and creativity in response to the changing needs of the clients and the organisation.
  • Proven ability to prioritise workload and meet set timelines, whilst working under minimal supervision.
  • Demonstrated competence in the relevant area of Nursing or Midwifery practice.
  • Demonstrated ability to relate to people from a diverse range of cultural and social backgrounds.
