About the role: 

Reporting to the Advanced Nurse Unit Manager, GEM at Home & Community Geriatrics, some of your duties will include:

• Implementing outcome measures appropriate to the client population to evaluate efficacy of therapy programmes in collaboration with the interdisciplinary team
• Independently assess client’s abilities, problems and needs using a variety of formal and informal assessments as needed
• Providing care planning, case management and liaison to ensure sustainable social support in place in community.
• Continually refining and evaluating department protocols, assessment formats, services and processes as part of the auditing/Service Improvement schedule.

About you:

• Ability to critically analyse or evaluate issues related to clinical care in social work in a manner which can be applied to quality activities, service evaluation or research.
• Experience providing care planning, liaison and case management.
• Extensive experience in aged care
• High levels of responsibility, initiative and organisational skills.
