About the Role

As part of the Central Adelaide Local Health Network (CALHN) Mental Health Clinical Program, the Social Worker (TOCC) plays a vital role in delivering recovery-focused services within a multidisciplinary team. You'll be supporting consumers across a variety of settings, including inpatient, sub-acute, community and rehabilitation programs for youth, adult and older persons. This role provides an excellent opportunity to apply your clinical knowledge and skills to support mental health recovery through care coordination, psychosocial and family assessments, evidence-based interventions and service planning.

Working from a recovery-oriented model of care, you will collaborate closely with consumers, their families and carers, GPs, NGOs and other community services to enhance wellbeing and promote sustainable community living. Your responsibilities will include facilitating the smooth and supported transfer of care from mental health services to community settings, while contributing to the development of targeted interventions and service improvement initiatives. About You

You are a compassionate and skilled Social Worker who brings a strong foundation in mental health practice and a commitment to recovery-oriented care. With a deep understanding of the complexities facing consumers with enduring mental illness, you are driven to make a positive impact through collaborative, evidence-based approaches.

  • Qualified and Accredited – Hold a recognised Social Work degree, eligible for full membership with the AASW, and have demonstrated clinical experience in mental health practice.

  • Clinically Proficient – Skilled in conducting biopsychosocial assessments, risk evaluations, and care planning for consumers with complex mental health and psychosocial needs.

  • Collaborative and Communicative – Effective in multidisciplinary teams and capable of engaging with consumers, carers, and external services to coordinate comprehensive care.

  • Ethical and Reflective – Work within AASW guidelines, participate in supervision, and commit to ongoing professional development and evidence-based practice.
