Are you a clinically experienced nurse ready to step into a leadership role where your work directly impacts patient care and wellbeing? At Port Pirie Regional Health Service, we’re looking for a Patient Journey Coordinator to lead complex discharge planning and ensure patients transition safely and smoothly from hospital to home or community services.
You’ll work with multidisciplinary teams, families, and external partners to support care continuity and better health outcomes. This is your opportunity to lead with compassion, coordination, and clinical insight — in a vital role that helps shape the future of regional healthcare.
What You’ll Do

Coordinate and lead complex discharge planning across clinical areas

Provide expert advice on care pathways and community-based supports

Support programs like Transition to Discharge and Out of Hospital Strategies

Collaborate with clinical teams to promote safe, person-centred care

Identify service gaps and lead quality improvement initiatives

Mentor staff and contribute to positive team culture
What We’re Looking For
-
Registration with the Nursing and Midwifery Board of Australia
-
At least 3 years post-registration experience
-
Strong leadership, communication, and problem-solving skills
-
A collaborative, person-centred approach to care
-
Experience in discharge planning, case coordination or clinical leadership
-
Knowledge of care standards, safety legislation, and service models
