About the Role

Make a real difference and be a valued member of the Eyre and Far North Local Health Network Community Nursing Team, based in beautiful Port Lincoln.

The Clinical Nurse – Respiratory Coordinator plays a pivotal role in the development, coordination, and delivery of high-quality respiratory services across the region. Working closely with multidisciplinary teams, this role supports improved health outcomes for people living with respiratory conditions through innovative, community-based care.

The position aims to:

  • Reduce emergency department presentations and hospital admissions for clients with respiratory conditions.
  • Support earlier discharge and reduce length of stay for patients admitted with respiratory illness.
  • Facilitate timely transfer of respiratory patients from metropolitan health services back to their community or residence.
  • Improve access to, and effective use of, health services for vulnerable populations.
  • Support patient education, self-management, and pulmonary rehabilitation to empower individuals to manage their health.
  • Deliver the SA Health Home Oxygen Program, including assessment, care planning, evaluation, and ongoing review.
About You
  • Do you hold current registration as a Nurse with the Nursing and Midwifery Board of Australia and bring at least three years of post-registration experience to your practice?
  • Are you a confident communicator and natural collaborator who thrives in multidisciplinary teams and can lead, influence, and support others?
  • Do you enjoy working autonomously while prioritising complex workloads and finding creative, flexible solutions to improve patient care?
  • Are you passionate about culturally safe care and committed to strengthening Aboriginal health outcomes through respectful engagement and continuous learning?
