Are you ready to lead meaningful care in the community? We’re looking for a passionate and organised Home Based Services Coordinator to join the Yorke and Northern Local Health Network in Wallaroo, supporting older South Australians to live independently in their own homes.
As the central link between clients, families, staff, and service providers, you’ll oversee the daily operations of home support services—including personal care, social support, and domestic assistance. Your work ensures people receive timely, respectful, and high-quality care aligned with funding and care standards.
Key Responsibilities

Coordinate referrals, assessments, service planning and scheduling

Lead and support Community Care Workers through rostering and supervision

Maintain accurate documentation, budgets and reporting

Liaise with palliative and transitional care teams to ensure seamless service delivery

Improve systems, support compliance, and contribute to team development
What We’re Looking For

Strong communication, time management and leadership skills

Experience in rostering, service coordination, and aged/community care

Knowledge of My Aged Care, CHSP, and relevant health sector standards

Proficient in using computer systems and managing sensitive information
