WHAT ARE WE OFFERING?
Join our team as an Optimal Ageing Care Coordinator on Kangaroo Island, delivering client-directed care to frail and older people. In this vital role, you will assess, plan, and coordinate care in clients’ homes to promote independence, wellbeing, and quality of life. You will supervise home support workers, liaise with clients, families, and professional staff, and ensure services meet standards while working within a supportive team environment.

The Barossa Hills Fleurieu Local Health Network runs from Kangaroo Island to Eudunda, wrapping all the way around our metropolitan areas (this position is on Kangaroo Island). - Coordinate and monitor services, prioritising referrals and managing waitlists.
- Conduct home-based assessments, develop care plans, and liaise with clients and families.
- Support, supervise, and provide professional development opportunities for home support workers.
- Maintain accurate records, ensure compliance with quality, safety, and reporting requirements.

WHAT ELSE DO YOU NEED TO KNOW/WHAT IS ESSENTIAL FOR SUCCESS?

- You must hold Community Service Certificate Level 3 or equivalent
