Join Our Team as a Community Support Worker – Support Independence, Your Way

The Eyre and Far North Local Health Network is embracing a Consumer Directed Care model for the delivery of aged care, disability, and support services—placing control directly in the hands of the people who use them. This innovative model allows consumers to choose the services they receive, when and how they are delivered, and who provides them.

As a Community Support Worker, you’ll play a vital role in helping people who are aged, frail, or living with disability maintain their independence—whether in their own homes or residential aged care settings. Your responsibilities will include personal care, cleaning, meal preparation, transport, and social support, all tailored to each individual’s needs and goals.

You’ll work collaboratively with consumers, their families, and health professionals to support quality outcomes and promote dignity and independence. To thrive in this role, you’ll be flexible, empathetic, self-motivated, and confident working independently in the community.

At Eyre and Far North LHN, we’re committed to your professional development. Ongoing training and support ensure you can grow your skills while delivering high-quality, person-centred care.

What We're Looking For:

  • Current drivers licence and willingness to drive.
  • Current Criminal History Check clearances (refer to Criminal History Check check clause).
  • Contactable by telephone.
  • Access to a reliable, registered and insured (minimum of third-party insurance) motor vehicle.
  • Meet immunisation requirements as outlined by the Immunisation Guidelines for Health Care Workers in South Australia 2014.
