About the Role

As a Dietitian within CALHN’s Integrated Care setting, you will play a key role in improving clinical outcomes for patients by delivering high-quality dietetic services across hospital avoidance, supported discharge, and hospital substitution models. Working closely with multidisciplinary teams, you will promote a ‘home first’ approach, providing assessment and interventions outside the acute hospital environment to support patients with complex needs.

Based with the CALHN BRIGHT service and working across CALHN sites as required, you will collaborate extensively with health, social, and community services to develop contemporary practice guidelines and care pathways. This role also involves contributing to education, quality improvement, and planning activities, while providing professional supervision to junior staff and students. It offers an exciting opportunity to work in a dynamic and evolving area that bridges primary and acute care.

About You

You are a qualified and motivated dietitian with a strong clinical background and a passion for delivering patient-centred care. Your ability to work independently and collaboratively in multidisciplinary teams enables you to manage complex cases effectively while contributing to service improvement and team development.

Key requirements:

  • Qualifications: Degree or equivalent qualification eligible for full membership of the Dietitians Association of Australia (DAA).

  • Clinical Skills and Experience: Proven experience in nutritional assessment, counselling, patient education, and managing clients with complex needs.

  • Communication and Problem-Solving: Excellent interpersonal skills, conflict resolution ability, and demonstrated initiative in addressing complex issues.

  • Knowledge and Innovation: Strong understanding of evidence-based practice, quality improvement principles, and the ability to supervise and mentor students effectively.
