About the Role
Are you a passionate senior physiotherapist looking to make a meaningful impact across the continuum of care? Join our dynamic Integrated Care (IC) team and be part of an innovative, multidisciplinary service connecting primary care with hospital specialties. This rewarding role focuses on providing culturally responsive assessments, clinical interventions and care coordination to clients with complex needs—across CALHN sites and beyond hospital walls. You'll champion a ‘home first’ philosophy, delivering multidisciplinary care in the community, supporting hospital avoidance and supported discharges.
As a valued clinician, you'll primarily assist in the management of clients with musculoskeletal conditions, recurrent falls, dizziness, or restricted mobility. Patients are often on a trajectory to an emergency department not requiring emergency level care, at risk of re-admission to hospital or admitted to Hospital In The Home. The Service operates over 7 days and regular weekend work will be required. The position works collaboratively with peers to contribute to Integrated Care’s service development, champion virtual care solutions, and in mentoring junior staff and students. Your input will help shape the future of Integrated Care pathways, bringing renewed focus to how allied health empowers individuals to thrive outside traditional hospital environments.
