About the Role
Join the Multidisciplinary Community Geriatric Service as a Social Worker and make a meaningful impact in the lives of older South Australians. In this rewarding role, you will apply your clinical expertise to support clients through comprehensive assessments, counselling, and person-centred care planning. You’ll work collaboratively with families, carers, and health professionals to empower older people, advocate for their needs, and promote autonomy through the least restrictive approaches.
You’ll be part of a dynamic, community-based team where no two days are the same. You’ll engage in therapeutic interventions, contribute to service improvement and research, and help deliver high-quality outcomes for people with complex care needs. About You
To be successful in this role, you will bring:

Eligibility for full membership with the Australian Association of Social Workers

Strong skills in client-centred assessment, counselling, and therapeutic planning

Experience supporting older clients with complex psychosocial needs
