    • AHP2: $88,255 - $102,240 p.a (pro rata) + superannuation + leave loading + salary sacrifice benefits
    • RN/RM2A: $103,353 - $110,497 p.a (pro rata) + superannuation + leave loading + salary sacrifice benefits
  • Ongoing part time (0.5FTE) over a 5 day roster

 

WHAT ARE WE OFFERING?

Join our dedicated Transitional Care Program team at Mount Barker, where you will provide short-term case management support for older people transitioning from hospital care. 

This is an opportunity to work as part of a progressive, values-driven team that supports older clients to maximise their independence and return home safely or make informed decisions about their ongoing care.

  • Deliver comprehensive case management and coordinate multidisciplinary care aligned to the needs of older clients.
  • Apply your professional expertise to assess, plan and evaluate individualised care plans.
  • Provide support, guidance, and clinical oversight to junior staff and students as appropriate.
  • Work collaboratively with families, carers, and service providers to ensure high-quality, culturally safe care.

WHAT ELSE DO YOU NEED TO KNOW/WHAT IS ESSENTIAL FOR SUCCESS?

Working in this environment, it is important to us that we only employ people with the upmost integrity and commitment to serving the South Australian community and its visitors to the highest possible standards.  To ensure that we meet that commitment there are a few checks that you will be required to undertake.  These are detailed on the Role Description, and do vary depending on what is required in the role, but are likely to include the following:

  • You will need to be immunised.  The policy on immunisation is very detailed, so please contact the Enquiries Person for this position if you have any queries.  Addressing vaccine preventable disease: Occupational assessment, screening and vaccination policy
  • You will need to undergo employment screening, what you require is also on the front page of the Role Description.
  • You will be required to hold registration with AHPRA to practice as a Registered Nurse or Registered Nurse/Midwife or an Allied Health Professional within our organisation.
